The failsafe or limp home mode on automatic transmission is when no electrical power is going to the A or B shift solenoids. This ca be caused by anything from a fuse or relay ( check you under hood fuse block) to a faulty transmission module (I think that is located under the center console toward the front). Start simple and check all your fuses and relays, then check the electrical connector at the trans and last would be you transmission module. Also be sure to check your grounds and if any aftermarket items have been added.
Hopes this helps

Rick

Is your check engine light on and if so are you able to scan it for DTC's, this may help in trouble shooting. You said that the power ckts are ok, but how are the grounds. Also the shift solenoids have power supplied at all times and are controled by the ground side of the ckt by the TCM (the same way fuel injectors are controled). Grounds are a little more difficult to trouble shoot. Your TCM may have gone bad since it is unlikely that both shift solenoids have gone bad at the same time and you stated that all the power ckts are good. But before you go and spend a couple hundred on a new TCM double check the ground control ckts from the TCM to the shift solenoids.

Hey I have a transmission book you should take a look at. Got it from when I took classes up in AK. It is very helpful if you want to borrow it you can.
